To file a claim against the City of Sacramento, you must first complete a Claim Form. Claim Forms are available for downloading and printing from this web page. They are also available at the City of Sacramento Office of the City Clerk, 1st floor of Historic City Hall, 915 I Street, Sacramento, CA 95814.
The City Clerk's Office is the only office to which claims against the City may be submitted.
Claims received by the Office of the City Clerk are logged and then forwarded to the City's Claims Administrator. Claimants are then notified that action will be taken within 45 days, or are otherwise notified regarding the claim itself.
Please carefully follow the instructions which accompany the claims form so missing information does not delay the processing of your claim.
